Built to simplify complex care journeys, it connects individuals, families and service providers with personalised information, services & peer support, helping people navigate life with greater clarity, confidence & dignity, no matter where they are on their path.

  • Living with a neurological condition often means navigating a fragmented system with little support. For 1in3 Western Australians affected, life beyond diagnosis is confusing, overwhelming & isolating. Existing resources are disjointed, duplicative or inaccessible. Our challenge was to co-design a digital solution with the neurological community that addressed the whole person, not just their medical needs. It needed to unify services, empower individuals, support carers and clinicians, and integrate without replacing what already works. Just as importantly, it had to create a compelling platform for investment making the case for long-term funding, engagement, and scalable impact across the broader health system.

  • NeuroHub was shaped through co-design workshops, lived experience interviews, sector consultations and iterative prototyping. The result is a modular, person-centred digital platform that connects people with neurological conditions to trusted information, services, support and practical tools. Rather than replicating clinical systems, NeuroHub bridges life and health supporting users in navigating housing, employment, relationships and care. The platform was designed for accessibility, neuro-inclusion and long-term scalability. Key roadblocks included unifying fragmented resources and building consensus across diverse stakeholder groups. Its strength lies in its foundation: a solution co-created with the community, validated by users, and built to evolve for real-world needs.

  • NeuroHub has already sparked a powerful movement: over 1,000 digital signatures, nine letters of support from peak organisations, and strong cross-sector momentum. Though still in concept phase, the platform has validated its critical need, attracted funding interest, and united a previously fragmented community. Its potential impact is significant. Simplifying access, reducing duplication, & empowering individuals, carers and clinicians alike. By connecting health and life pathways, it challenges conventional models of care and offers a blueprint for scalable, person-led innovation. NeuroHub is more than a product it’s a catalyst for system change, with potential to influence policy and practice far beyond WA.

  • NeuroHub is a modular, person-centred platform co-designed with over 70 individuals from the neurological community, including people with lived experience, carers, clinicians and service providers. This deep, participatory process shaped every element of the design ensuring the platform is not only functional, but trusted, inclusive and representative. Unlike clinical portals, NeuroHub takes a whole-of-life approach. It connects users to verified information, local services, research opportunities and peer support addressing needs across health, home, work, relationships and care. The platform is built to be intuitive, accessible and neuro-inclusive, with plain language, calm visual design and multiple content formats supporting a diverse user base. The architecture is modular and scalable, allowing for integration with health, disability and aged care systems over time. Rather than replicating existing services, NeuroHub complements them filling critical gaps in navigation, coordination and continuity of care. A key design principle was advocacy-readiness. The platform is paired with a strategic data and storytelling framework, enabling funders and policy partners to see its impact clearly. This approach has already sparked significant public support, sector alignment and investment interest. NeuroHub sets a new benchmark for platform-based health equity designed not just to inform, but to empower, connect and transform.
